SS_PB 1.1 Describe the physiological principles relevant to optimising blood flow to tissue flaps, including:
Autonomic nervous system control of systemic vascular
  • The sympathetic nervous system (SNS) plays a dominant role in vascular tone regulation via α1-adrenoceptor-mediated vasoconstriction and β2-adrenoceptor-mediated vasodilation.
  • Parasympathetic nervous system (PSNS) has minimal direct control over systemic vasculature but can influence heart rate and indirectly impact perfusion.
  • Baroreceptor reflexes help maintain systemic blood pressure and perfusion.
  • Surgical and anaesthetic implications: SNS activation (stress, hypothermia, pain) leads to vasoconstriction and reduced flap perfusion. Anaesthetic techniques, such as regional anaesthesia, can block SNS activity and improve blood flow.
