Pulmonary contusion
  • Potentially life threatening blunt trauma
  • Respiratory failure develops the injured lung parenchyma develops an inflammatory process leading to oedema and bleeding, compounded by pain and paradoxical chest wall movement -> hypoxic respiratory failure
  • Predictors of severe pulmonary contusion:
    • CXR evidence
    • 3 or more rib #’s
    • Chest drain
    • Hypoxia at admission
  • Management:
    • Oxygen
    • Improving FRC (chest PT, NIV with CPAP, IPPV with PEEP)
    • Analgesia
    • Careful fluid balance
