- Respiratory failure develops the injured lung parenchyma develops an inflammatory process leading to oedema and bleeding, compounded by pain and paradoxical chest wall movement -> hypoxic respiratory failure
- Predictors of severe pulmonary contusion:
- CXR evidence
- 3 or more rib #’s
- Chest drain
- Hypoxia at admission
- Management:
- Oxygen
- Improving FRC (chest PT, NIV with CPAP, IPPV with PEEP)
- Analgesia
- Careful fluid balance
