SS_TS 1.21 Outline the management of the following postoperative complications associated with thoracic surgery: 
- Nerve injury 
Can result from compression, stretch or direct injury. 
  • History + examination : determine time course and identify pre-existing neuropathy.
    • Clinical exam determines if multiple nerves involved and sensory or motor  component or both which is of prognostic value. 
    • If deficit found EMG and imaging + consultation with neurologist. 
  • Can consider phrenic nerve injury in difficult wean post-operatively. Most well patients cope with paralysed hemidiaphragm but thoracic patients often have poor respiratory baseline. 
    • Most self limiting, if severe consider diaphragmatic pacing.
