What are some red flags to think about in a child with fever?
  • Ill appearing on physical exam
  • ALL neonates < 28 days (4 weeks)
  • HSV features esp if maternal risk factors (mucocutaneous vesicles, seizures, focal neuro findings)
  • Neonates < 3 mos with clinical or lab features that suggest bacterial infx
  • Lack of immunization
  • Inconsolable
  • Resp distress (decreased O2 sat, nostril flaring, grunting, intercostal or subcostal indrawing, supraclavicular or suprasternal indrawing/ retraction, scalene m use (palpation), adventitious sounds)
  • Lethargy, listlessness, toxic appearance
  • Petechiae, purpura
  • Non-blanching rash
  • Travel history
  • Chronic fever, FoUO
  • True hypotension
