What is the typical electrolyte derrangement for vomiting?
Hypovolemia
Hyponatremia (less common)
  • Vomiting --> dehydration/ volume depletion --> ADH --> water retention and dilutional hyponatremia
  • Direct loss (less than others)
Hypokalemia
  • Direct loss
  • Due to acquired alkalosis --> kidneys excrete more potassium in exchange for H+ ions
Hypochloremia
  • Loss of gastric contents (i.e. HCl) --> depletion of chloride ions
Metabolic alkalosis
  • Hydrogen ions secreted into gastric lumen --> lost due to vomiting --> relative increase in HCO3-
  • Loss of chloride --> kidneys compensate to ensure electrochemical homeostasis --> retention of bicarbonate
