Perineum, GI tract, female GU tract, axilla
- Emperic, mild: Cephalexin 500-1000 mg oral QID for 2-5 days
- Empiric, moderate-severe: Cephazolin 2 g IV q8h for 5-10 days
Perineum, GI tract, female GU tract, axilla
- Empiric, mild: Amoxicillin-clavanulate 875-125 mg oral BID for 5-10 days
- Empiric, mild, MRSA suspected: 875-125 mg oral BID for 5-10 days + TMP SMX 1-2 DS tabs oral BID
- Empiric, moderate: Cefazolin 2 g IV q8h for 5-10 days + metronidazole 500 mg oral BID
- Empiric, severe: Piperacillin-tazobactam 3.375 g IV q6h for 5-10 days
- Empiric, severe, MRSA suspected: Piperacillin-tazobactam 3.375 g IV q6h for 5-10 days + vancomycin 15 mg/kg IV q12h
