What are some principles of fluid resuscitation in a trauma patient?
Minimize IV fluid. Give IV F only for resus of hypotensive (e.g. MAP < 65) and then only until blood is available; and only if fluid responsive (i.e. SV increaseds by at least 10% after 500 mL of crystalloid).

Initiation of massive transfusion protocol based on assessment of blood consumption score. >=2 predicts need for massive transfusion (sn 75%, sp 86%).
  • penetrating mechanism of injury
  • Positive FAST
  • SBP 90 mmHg or less
  • HR > 120
Blood products should be used in equivalent ratios or whole blood if available.
