What is a mnemonic for causes of anion gap metabolic acidosis?
If the gap is enlarge, then there is some other unmeasured anion in the blood caused by excessive organic acid production (products are H+ and unmeasured anion).

Common causes of this are:
  • Metabolic acid generation (e.g. lactic acidosis, ketoacidosis)
  • Ingestions (e.g. ASA, ethylene glycol, methanol)
  • Accumulations (e.g. reduced GFR)

  • Methanol ingestion (formic acid)
  • Metformin (L-lactic acid)
  • Uremia (retention of SO4 and PO4)
  • Diabetic ketoacidosis (beta-hydroxybutyrate ketones +/- lactate)
  • Paraldehyde ingestion (acetic acid)
  • Isoniazid (ketones)
  • Isopropyl alcohol ingestion (formate and acetate via acetone)
  • Lactate (L-lactate, D-lactate)
  • Ethylene glycol ingestion (glycolic, glyceric, and oxalic acid)
