year old man who has recently returned from a trip to Thailand presents to the
emergency room with 5 days of diarrhoea, nausea and right upper quadrant
abdominal pain.
On examination he is febrile hypotensive (systolic BP 110mmHg)
and tachycardic (110 beats per minute).
