You are a junior registrar in a surgical unit. You are operating with the fellow today, doing an inguinal hernia repair. You have seen and consented the patient in the outpatient clinic, in the presence of his family (who are known to be quite difficult and litigious). The patient is known to have bilateral hernias. The operation is elective for a symptomatic (but not incarcerated) hernia.
The operation goes as planned, but afterwards, it becomes evident that you have operated on the wrong (asymptomatic) side. A final check (team time out) was not performed prior to commencing the operation.
The fellow, who was the operating surgeon, asks you to go and break the news to the family.
